In a small town, there's a lot of gossip because people know each other well. The expression is definitely not the same as "Mr. America" or "Ms. America" -- which, by the way, are not common expressions.

But "all over America" sounds 'natural' enough to me - and means that it is being shown everywhere in America - not just in one theatre (as could be the case if you merely say 'in America).
