Unity itself, cannot produce a busy signal. It can however, produce a situation where the PBX would return busy signal to the caller: all of its ports are busy.

You have to check what number is being dialed and maybe the issue is on the remote end - nothing much you can do at that point.

I expect they are either a) not telling you the call is ending and just playing busy inband instead; or, b) sending you a disconnect with the wrong cause code.
