The expression is used colloquially and jocularly among friends. I wouldn’t use it elsewhere.

I think "Keys to the Cave" implies that all men are cave men/neanderthals, and women need a key/guide to help them understand how men think. There was a one-man Broadway show that was very popular many years ago called "Defending the Cave Man." It was about the differences between men and women.
