Personally I would also capitalise "my Aunt Fanny" - she is "Aunt Fanny" to me (although, of course, to you, she is Mrs Blenkinsop) - but it is also possible to read it as "my aunt (who is called) Fanny".

Ils sont sympathiques -> ils sont sympa. That said, when abbreviations become so common that they are massively considered as full words, they act as such and, then, agree : Ils sont sympas.
